COVID-19 Employee Daily Health Questionnaire

The Health and Safety of employees is the highest concern for businesses. It may be necessary to keep track of employees health and any symptoms they may be experiencing that are associated with COVID-19. Employees would be required to complete the Form at the beginning of each workday.

ECS Imaging has developed this online forms process for it’s own employees to indicate their Workplace Entrance Status for Covid-19?  The Form allows them to indicate if they have experienced any of the symptoms associated with COVID-19. Additionally, this form can be used by any organization.

Here is the form & business process in 3 easy steps:

1. The employee fills out this electronic form:

1. Once the form is filled out it is routed to the HR person for acknowledgment.
2. At the same time this form is automatically filed into the Employee’s individual HR file in Laserfiche.
